Bring or send any labs you’ve had checked recently, and it may be recommended to request additional lab work at your next doctor’s appointment. Also gather together all medications and supplements you are taking.

You may also be asked to fill out a baseline assessment form if you are dealing with issues like gut health or depression. This can help track your symptoms as changes are made to your diet and lifestyle so you can see how you are progressing.
